Section 13 &39 Name in your own language or script <br />这个一定要父母签名么？<br /><br />2. Section 71 How will you pay your application charge?<br />Credit Card Number 填完16位数之后，还有3个空格，这个三个空格是要填写卡背后的CVV吗？<br /><br /><br />3. 关于父母与子女的关系证明，请问需要准备什么材料？<br />我有一个户口本公证，但是上面的信息都是很早之前的了，我的资料显示我还在高中，不知道这个户口本公证可以用作算父母与子女的关系证明吗？<br /><br />4. 父母在国内，需要护照公证吗？<br />听朋友说第一次递交的时候，只需要交Form 47PA, 40, 956， 签证费，和护照复印件就行，求证实！谢过！<br /><br />5. 补问： <br />From47PA第25页的checklist第二项是说birth registration：<br /><br />Documents to prove your identity – a certified copy of your birth registration showing both parents’ names. Documents to prove your identity – a certified copy of your birth registration showing both parents’ names. If you do not have a birth certificate or are unable to get one, you must provide a certified copy of the identification pages of at least one of the following documents:
• passport
• family book showing both parents’ names
• identification document issued by the government
• document issued by a court that verifies your identity.
