Certified copies of birth certificates showing both parents’ names for you and any other person included in the application. If you do not have a birth certificate and are unable to get one, you must provide a certified copy of the identification pages of at least one of the following documents:

爷爷奶奶外公外婆的户口在同一个城市吗？ 如果两家不在同一个城市，就要去省公证处办了。我父母刚办了，两家户口不在同一个城市，在同一省。不在世了，派出所可以出死亡证明。要是太早了，就不好说了。我外公是87年没得。我家里人拿着骨灰盒保存证去证明的。作者: wanghe410 时间: 2015-5-21 14:56

From47PA第25页的checklist第二项是说birth registration：

第五项仍然要求birth certificate：
Certified copies of birth certificates of all persons included in the application.
