话说表格好多内容要填，求过来人解答>.<

关于Form 47PA
1. Section 13 &39 Name in your own language or script
这个一定要父母签名么？

2. Section 71 How will you pay your application charge?
Credit Card Number 填完16位数之后，还有3个空格，这个三个空格是要填写卡背后的CVV吗？

3. 关于父母与子女的关系证明，请问需要准备什么材料？
我有一个户口本公证，但是上面的信息都是很早之前的了，我的资料显示我还在高中，不知道这个户口本公证可以用作算父母与子女的关系证明吗？

4. 父母在国内，需要护照公证吗？
听朋友说第一次递交的时候，只需要交Form 47PA, 40, 956，签证费，和护照复印件就行，求证实！谢过！

5. 补问：
From47PA第25页的checklist第二项是说birth registration：

Documents to prove your identity – a certified copy of your birth registration showing both parents’ names. If you do not have a birth certificate or are unable to get one, you must provide a certified copy of the identification pages of at least one of the following documents:
• passport
• family book showing both parents’ names
• identification document issued by the government
• document issued by a court that verifies your identity.
If you are unable to provide one of these documents, you must provide other acceptable evidence that you are who you claim to be.

第五项仍然要求birth certificate：

Certified copies of birth certificates of all persons included in the application.

这个可以用护照公证代替吗？

不是签名，自己填

不知道，我用bank cheque

我用的是国内公证处的子女关系公证，觉得这个比较保险，户口簿不知道顶不顶用，不过你可以试试。

父母在国内，护照公证不需要，复印件即可。
没听说过交这么少材料就行的，不过你实在赶不及可以试试。
